Be a part of a national conversation that brings medical professionals and community members together. Learn how to have important conversations ahead of a medical crisis or serious illness. Such a conversation can provide a shared understanding of what matters most to you and your loved ones. Advanced care planning can improve quality of life and help in making difficult decisions. An expert panel composed of an oncologist, geriatric primary care physician, hospice nurse, and chaplain will engage the audience in a conversation on how to take concrete steps to identify and communicate wishes for end-of-life goals and care preferences.
